=Situation=
65M D4 admission with mixed ADHF (CAP and hypervolaemia); improving. Reduced oxygen requirement, negative fluid balance, improving inflammatory markers, euvolaemic and at dry weight. Planned for discharge today.

=Subjective=
Feels great
Keen for discharge.
No dyspnoea at rest.
Mobilising in corridor without difficulty.
No chest pain overnight. Sleeping flatter. Eating well, barriers resolved.
Cleared by PT yesterday. Fully independent.
No allied health barriers to discharge.

=Objective=
Vitals: T 36.5, HR 80, BP 118/70, RR 16, SpO2 95% RA.
Weight: Current weight: 78.6kg, Target weight: 79kg
Fluid balance: Input: 1.2L Output: ~2L Balance: -0.8L
Examination findings: Comfortable, no increased work of breathing, faint residual right basal crackles. JVP not elevated, oedema resolved, cardiovascularly stable.
Results:
Bloods //
WCC 10.7, Na 139, Cr 280, K 6.1
Micro //
Blood cultures no growth. Sputum not obtained.
Imaging //
Nil new.

New severe AKI with Hyperkalaemia
